Intraoperative photos, schematic drawings, MRI, CT, 3D angiography, and computer rendered images help the authors in a very useful guide for residents, fellows, and neurosurgeons. The surgical approaches include transsphenoidal, transmastoid, retrosigmoid, suboccipital, retrolabyrinthine, petrousal, subfrontal, etc. are diagrammed with a step by step guide to complex pathologies. EC IC bypass, vascular decompressions, IC-IC, STA-MCA, and saphenous intracranial grafting are all illustrated well. Giant aneurysms, DAV malformations, carotid cavernous fistulae, and giant arterial venous malformations are covered including operative techniques. Kawase’s, Parkinson’s, Mullan’s, Fukushima’s, Yarsargil’s, Spetzger’s etc. triangles are illustrated.
