This article reviews the key pathogenic mechanisms underlying the development of central nervous system autoimmunity. It covers non-modifiable risk factors (age, sex, ethnicity, genetics), interactions between environmental factors and epigenetics, triggers associated with the loss of immune tolerance (infections, tumors, immune-checkpoint inhibitors), advances in understanding lymphatic drainage and neuroimmune interfaces, and novel therapeutic approaches based on immunopathogenesis knowledge.
