This review discusses recent advances in understanding paraneoplastic syndromes (PNSs) in neuro-ophthalmology, focusing on the immune response triggered by aberrant expression of onconeuronal antigens. It highlights the use of FDG/PET-CT scanning in diagnosing occult tumors, new antibody associations (e.g., anti-CV2/CRMP-5), identification of novel antigens associated with melanoma-associated retinopathy (MAR), and updated treatment options including calcium-channel blockers, alemtuzumab, and rituximab. The necessity of a high index of suspicion for early diagnosis and improved prognosis is emphasized.
