This review provides an overview of a physiology-based approach for assessing the respiratory system of patients receiving invasive mechanical ventilation at the bedside. It discusses extensive and intensive ventilatory variables, methods to evaluate both passive and spontaneously breathing patients, and aims to guide clinicians in setting mechanical support to achieve lung and diaphragm protective ventilation.
