This study evaluates the test characteristics of serum alpha-fetoprotein (AFP) for hepatocellular carcinoma (HCC) screening, noting the increasing rates of alcohol-associated liver disease (ALD) and metabolic dysfunction–associated steatotic liver disease (MASLD). The findings suggest that lowering the AFP threshold from ≥20 to ≥10 ng/mL improves test sensitivity while maintaining high test specificity across various liver disease etiologies.
