This mini-review discusses the key role of microcirculatory dysfunction in sepsis and the use of sublingual videomicroscopy for real-time, non-invasive assessment. It highlights the potential loss of ‘hemodynamic coherence’ during traditional resuscitation and advocates for integrating microvascular monitoring to guide and titrate resuscitative therapies in septic shock patients.
