This observational cohort study aimed to compare the long-term prognostic value of artificial intelligence–guided quantitative computed tomography ischemia (AI-QCTischemia) with hybrid coronary computed tomography angiography/positron emission tomography (CCTA/PET) myocardial perfusion imaging in suspected coronary artery disease (CAD). Symptomatic patients underwent CCTA with selective downstream PET. Results showed moderate-to-substantial agreement between the methods. Both ischemic CAD on hybrid CCTA/PET (HR, 2.35) and abnormal AI-QCTischemia (adjusted HR, 1.98) were independently predictive of adverse outcomes (death, MI, or unstable angina). The study concludes that the AI-QCTischemia algorithm demonstrated long-term prognostic value comparable to hybrid CCTA/PET perfusion imaging in suspected CAD.
