This original article quantifies surgical access to the Internal Auditory Canal (IAC) using an exclusively endoscopic transcanal approach (EETA) based on anatomical dissection of temporal bones and subsequent CT analysis. The study successfully achieved access and visualization of the labyrinthine segment of the facial nerve in all specimens. It confirmed that EETA can safely access the entire IAC, maintaining safe distances from key neurovascular structures (carotid artery, jugular bulb, middle fossa), offering a minimally invasive alternative for patients without serviceable hearing who require removal of intrameatal and medial IAC tumors.
